COVID-19 LIABILITY ACKNOWLEDGMENT
Due to the 2020 outbreak of the novel coronavirus (COVID-19), we are taking extra precautions with the care of everyone we interact with to include health history review and temperature check with all parties.
Despite these extra precautions, we cannot prevent those from becoming exposed to, contracting, or spreading COVID-19, and therefore, understands that by interacting with NARRATVES during this time, there is an increase risk to exposure, contracting, and/or spreading COVID-19.
Symptoms of COVID-19 include:
- fever
- fatigue
- dry cough
- difficulty breathing
I agree to the following:
- I understand the above symptoms and affirm that I, as well as all household members, do not currently have, nor have experienced the symptoms listed above within the last 14 days.
- I affirm the I, as well as all household members, have not been diagnosed with COVID-19 within the past 30 days
- I affirm that I, as well as all household members, have not knowingly been exposed to anyone diagnosed with COVID-19 within the past 30 days.
- I understand that NARRATVES cannot be held liable for any exposure to the COVID-19 virus caused by misinformation on this form or the health history provided by each client.
Our business is following these enhanced procedures to prevent the spread of COVID-19 based on the recommendations of the CDC:
- We will limit the number of people we photograph to no more than 10 people
- We will maintain a 6 foot distance between you and our staff. Please help us by doing the same.
- Although we like you, we will not shake hands or greet you in any manner that requires physical contact.
- We will not allow visitors to the session, we need to avoid unnecessary contact with people not directly involved in the photo session.
- We will sanitize all our equipment before each session and we ask that you do not touch or handle any of our gear.
- We will also either wash our hands thoroughly and/or use hand sanitizer before and after each session.
Please be flexible, as we will adapt as we are given new information as they emerge.
WAIVER OF LIABILITY
I, on behalf of myself, my family, estate, heirs, successors, and assigns hereby forever release and waive my right to bring suit against NARRATVES and its owners/agents and representatives (collectively the “released parties”) from any and all liability, claims, demands, actions, and causes of action, directly or indirectly arising out of or related to any loss, damage, injury, or death in connection with exposure, infection, and/or spread of COVID-19 whether caused by the negligence of the released parties or otherwise while interacting with NARRATVES for any purpose including interviewing, recording, and photographing participants for NARRATVES projects or any scope of work deemed.
From NARRATVES choice of law and severe-ability: I agree that this waiver of liability shall be governed by and construed in accordance with the laws of the state of California, without regard to conflict-of-law principles. If any provision of this waiver is deemed to be invalid or unenforceable in any respect for any reason, the validity and enforceability of such provision in any other respect and of the remaining provisions of this waiver will not be impaired in any way. I have read, understood, and agree to the terms of this waiver.
